2. Mechanism

Substantia nigra degeneration occurs in patients with Parkinson's disease. This condition leads to disruption of the nigrostriatal pathway, which reduces striatal dopamine levels. Different from dopamine dopamine, levodopa can cross the blood-brain barrier (BBB). Levodopa is converted to dopamine both in the central and peripheral nervous system. To increase the bioavailability of levodopa and reduce its side effects, it is often used in combination with peripheral decarboxylase inhibitors such as carbidopa and benserazide. Dopamine decarboxylase inhibitors prevent the peripheral conversion of levodopa to dopamine, thereby allowing more levodopa to cross the blood-brain barrier. Once converted into dopamine, it activates postsynaptic dopamine receptors and compensates for the decrease in endogenous dopamine.
